Our department​ provides this web page as a resource for answering frequently asked questions and providing the necessary documentation for records requests regarding EMS Billing Services.

Obtaining A Copy of an EMS Report

To obtain a copy of an EMS report, a​ Protected Health Information Release and Affidavit must be completed and submitted to the Charlotte County Fire & EMS Billing Division.

This form may be notarized or accompanied by a valid photo ID. Completed forms can be faxed or mailed to the below address:

Charlotte County does not charge patients obtaining their own medical records. For other outside requests, as per Florida State Statute 456.057, Rule 64B8-10.003, we charge $1.00 per page up to 25 pages and $0.25 per page after that.
